This policy explains what Maxivita Insurance LLC (“we”) collects through maxivitainsurance.com, why, who we share it with, and the choices you have. Our separate GLBA privacy notice describes how we handle nonpublic personal financial information in connection with insurance services; where the two overlap, the GLBA notice governs that information.
Information we collect
Information you give us
- Contact details: name, email address, telephone number, state of residence.
- Information needed to look at coverage: age, the type of coverage you are interested in, and anything you choose to write in a free-text field.
- For agent applicants: licensing status and background information you provide.
- Records of your consent: the exact consent text displayed to you, the date and time of submission, the page it was submitted from, and technical details of the submission. We retain this to demonstrate consent.
Information collected automatically
- Standard server log data: IP address, browser type, pages requested, and referring page.
- Advertising and analytics identifiers set through cookies and similar technologies, including tools operated by advertising platforms, used to measure the performance of our advertising and to show ads to people who have visited this site.
What we do not want
Please do not submit medical records, diagnoses, Social Security numbers, or financial account numbers through a web form or by email. If health information is needed for an application, your agent will collect it directly through the carrier's secure process.
How we use information
- To have a licensed agent contact you about the coverage you asked about.
- To prepare and submit an insurance application to a carrier at your direction.
- To service a policy after it is issued, and to assist your beneficiaries at claim time.
- To evaluate agent applications.
- To measure and improve our advertising and this website.
- To comply with law, including insurance recordkeeping and telemarketing consent recordkeeping.
Who we share information with
- Insurance carriers and our national brokerage, to obtain quotes, submit applications, and service policies.
- Our licensed agents, who are the people who contact you.
- Service providers that host our systems, deliver email and text messages, and operate our customer records, under contracts limiting their use of the information.
- Advertising platforms, which receive online identifiers through cookies and similar technologies for advertising measurement and targeting. Under some state privacy laws this is considered “sharing” for cross-context behavioral advertising. See your privacy choices.
- When required by law, or to protect our rights.
We do not sell your name, telephone number, or email address to unrelated marketing companies or lead buyers.
Cookies and tracking
We use cookies and similar technologies for site functionality, analytics, and advertising measurement. You can control cookies through your browser settings. We honor the Global Privacy Control (GPC) browser signal as a request to opt out of the sharing of personal information for cross-context behavioral advertising where applicable law requires it.

- Stop calls and texts. Reply STOP to any text message. Ask any agent to add you to our internal do not call list, or email compliance@maxivita.io with the number. You may revoke consent at any time by any reasonable method, and we will honor it promptly.
- Stop emails. Use the unsubscribe link in any marketing email.
- Access, correction, and deletion. Depending on your state, you may have the right to request access to, correction of, or deletion of personal information, and to appeal a denial. Contact us using the details below. We will verify your identity before acting on a request, and we will not discriminate against you for exercising a privacy right.
- Opt out of sale or sharing. See your privacy choices.
Retention and security
We keep information for as long as needed for the purposes above and for the periods required by insurance, advertising, and telemarketing recordkeeping rules — for consent records and advertising materials, generally at least five years. We maintain administrative, technical, and physical safeguards designed to protect personal information, though no method of transmission or storage is completely secure.
Children
This website is not directed to children under 16, and we do not knowingly collect their personal information.
Changes
We may update this policy. The date at the top reflects the current version, and material changes will be reflected here.
